Transition to Adult Services

Many young people experience disruptions in care during this transition, with only about half of those who need ongoing support successfully transfer to adult services. To improve outcomes, experts recommend starting transition planning early (at least 6 months before the transfer), providing young people with a named coordinator to help navigate the process, and ensuring there is a written transition plan that outlines ongoing treatment needs and support.
