Discharge
The post-discharge period is an extremely vulnerable time for children and adolescents leaving inpatient mental health services, with increased risks of readmission and adverse outcomes. Effective discharge interventions typically include elements like risk assessment, individualised care planning, discharge preparation, community linkage, psychoeducation, and follow-up support. While discharge interventions from inpatient child and adolescent mental health services vary in structure, they consistently play an important role in minimising patient and family vulnerability after discharge.
